If you are not a member of HPCANYS, the annual licensing fee is $4000. We are anticipating that the second year license will be about half the first year fee. Who developed Assuring Your Wishes?Assuringyourwishes.org was developed by United Hospice of Rockland, Inc.(UHR). Since 1988, UHR has provided care, hope, comfort and improved quality of life to individuals and their families facing serious illness. We offer compassionate support to members of our community who have experienced the loss of a loved one. We lead the health and human services community in improving the provision of care to those affected by serious illness. It was only natural that our experiences would lead us to develop Assuringyourwishes.org. AYW will shortly become its own not-for-profit organization. What is the benefit of purchasing a license for www.Assuringyourwishes.org?The program is designed for any organization that believes in the importance of advance care planning and/or needs to be able to access individuals’ advance directives. www.Assuringyourwishes.org enables you to maintain visibility in your community while encouraging conversations between family members about personal choices, enhancing consumer understanding about advance care planning and facilitating continuity of care across settings. www.Assuringyourwishes.org is perfect for hospices, nursing homes, hospitals, medical societies, and other community organizations. Who can view directives that are stored on www.Assuringyourwishes.org?(AYW)When an individual stores his/her directives, they are given three ID cards with their name and password. One is for them to carry in their wallet, one is to give to their health care agent and one is for their physician. They can also request additional copies to give to others. Physicians who have registered with the AYW (registration is done on-line) can search for directives by entering their name and password and their patients’ names and dates of birth. Health care organizations having a need to access directives can be given a password by the local licensee of AYW. What does the package include?The complete starter kit includes the website, physician and consumer databases, member ID cards, customized brochures and technical support. Do I have to buy the marketing materials?The marketing materials are not required for purchase. They are sold individually and may be helpful for you to promote the program to your community.
