PROGRAM OVERVIEW
www.Assuringyourwishes.org provides information and instructions on the completion of advance directives. It enables users (and their health care agents and physicians) to securely store and retrieve their documents-by the click of a mouse--whenever needed. The overall objective of www.Assuringyourwishes.org is to ensure that individuals' health care directives are documented, known to their loved ones and physicians, accessible whenever and wherever needed, and most importantly, respected.
By encouraging members of your community to complete health care proxies, living wills and organ donation wishes and to submit them to www.Assuringyourwishes.org., you will be offering your community an invaluable service. Too often, advance directives (documents such as a health care proxy and living will that outline patient wishes and designate a decision maker) are nowhere to be found when they are most needed. You can prevent that from happening by offering your community members the opportunity to file their advance directives with www.Assuringyourwishes.org.
The key to success is a highly secure and sophisticated, but user friendly, website into which the documents are scanned and stored. Individuals are issued a computer generated password with ID cards. The individual is instructed to carry the ID card in their wallet, give one to their health care agent and one to their physician. Physicians and health care agents are then able to access a participant's advance directives by going to the website and entering the participant's password. Physicians registered with the site can check if their patients’ directives are stored by entering their name and self-selected password, their patient’s name and their date of birth.
We have made it easy for you by providing a turn-key service. We offer everything you need to ensure a successful program.
The starter kit includes:
- A license to use a customized version of www.Assuringyourwishes.org in your county
- Databases to maintain demographic information of those storing directives and one for physicians registering with the site

- 500 (3 part) Member Identification Cards to be sent to those who store their directives
- 500 9” X 12” envelopes
- 500 Customized Program Brochures
Although this service is designed so that users do not have to pay to store their directives, we have found that people often choose to make a tax-deductible donation for this complimentary service when given the opportunity to do so.
